Regarding students in Grade 10 for 2015-16:
Please provide the nurse with a recent physical exam report
from their Primary Care Provider between now and when school begins in September. This should include an updated
immunization record. These can be mailed to me or dropped off at school. I will be conducting hearing and vision
screenings on the tenth graders next school year.
Please keep the school nurse informed of any changes throughout the coming year regarding your child’s health, such as
diagnosed infections, illnesses, injuries and medications taken on a regular basis.
